A large-scale observational study in the United States (MrOS for elderly men with osteoporotic fractures, MrOS) showed that elderly men taking selective serotonin reuptake inhibitors (SSRI) caused a decrease in bone mineral density (BMD). This is not the case with other antidepressants.
Studies have shown that SSRI is associated with decreased bone density. It is unclear whether depression itself or a drug for depression causes a decrease in BMD in patients.
To investigate whether SSRI was associated with low BMD in older male patients, the team conducted a prospective MrOS study. From 2000 to 2002, 5,995 elderly patients (age ≥ 65 years) from 6 regions of the United States were included. The participants were divided into the untreated group (5708 cases), the SSRI group (160 cases), the tricyclic antidepressant (TCA) group (99 cases), and the trazodone hydrochloride group (52 cases) according to the antidepressant drugs. The primary endpoint was total hip (femoral and femoral neck) and lumbar spine BMD.
RESULTS: The investigators found that SSRI use was associated with a lower mean BMD (hip and spine), but there was no significant difference in bone density between the TCA group, the trazodone group, and the untreated group.
The main investigator, Dr. Haney of the University of Minnesota, said the study suggests that low BMD is associated with taking SSRI. This is also supported by in vitro and animal studies, but further prospective studies are needed to confirm this.
Once confirmed, SSRI users may need to perform osteoporosis screening and take precautions.
